It identifies Foreman as instructor aboard Wellington III X3338 of No. 12 Operational Training Unit, coded JP-P, on a bombing-practice flight from Chipping Warden. The aircraft crashed about two miles north-east of Woodford, Northamptonshire, at about 16:30 on 13 January 1943. Foreman, pupil pilot Sergeant Gordon Campain, Pilot Officer Dudley William Charles Jepp, Sergeant Benny Sober and Sergeant William Hugh John Nott were killed, while one air gunner was injured.
